If you plan on filing an application for the Civil Service Exam - may it be the CSE-PPT (Career Service Exam - Pen and Paper Test) or the COMEX, here's a list of things you have to prepare: Fully accomplished Application Form (CS Form No. 100 (Revised November 2012)). You can download the form from the CSC website and print it out. You can also get the form directly from wherever CSC office you intend to file an application. When filling it out, remember that the spaces for “Signature of Applicant” and “Right Thumbmark” on the form should be left blank. These shall be accomplished in the presence of the CSC processor.
